The Clinic Management Collection
A complete set of professionally developed clinical forms for patient charting and management (and, you can customize the forms with your clinic's name).
- New Patient Information (4-form package)
- A group of four forms to be filled-in by the patient prior to initial intake:
- Patient Information (general identification information)
- Reason for Visit (background and symptom survey)
- Medical Information (medications, conditions, history, etc.)
- Statement of Informed Consent (patient agreement)
- Patient Intake
- Use this form to document patient intake question and observation interview, i.e.; "ten" questions, tongue, pulse, diagnosis and treatment.
- Merdian Acupuncture
- Use this form during patient intake to indicate the pathology area in relation to the meridians. Identify type; pain, numbness/tingling, weakness. Indicate location of pathology on the appropriate illustrated views of the body and head.
- OB/GYN Information
- Use this form for gathering obstetric and gynecologic information.
- Pediatric Information
- Use this form to document pediatric patient information; pregnancy and birth, development, diet/lifestyle, immunizations.
